This Pharmacist position is within an interdisciplinary and progressive regional practice with clinical, education, and research/quality improvement responsibilities.
Position description
Contributes to a regional and interdisciplinary healthcare team as a proactive member of the group in providing patient centered, timely, equitable, efficient, safe, and effective healthcare. Understands all activities related to order entry and review of medication orders and displays proficiency in understanding how it relates to technological interfaces and generation of the patient medication administration record. Ensures safe, appropriate, and cost-effective medication therapies for patients by monitoring drug levels and therapeutic drug regimens. Provides and obtains medication information to/from patients and their caregivers or families in a timely and professional manner. Oversees all the activities of pharmacy technicians, fosters an environment of mutual respect and contributes to efficient, safe, and timely operations of the central pharmacy distribution area. Contributes to educational activities involving pharmacy staff and medical and pharmacy residents/students. Collects, monitors and maintains all necessary medication use records and assists in maintaining the approved medication formulary in appropriate inventory levels. Maintains a current level of professional competency and assumes personal responsibility for self-development and ongoing education to maintain proficiency. Encourages timely and safe patient care by responding to emergent situations calmly and efficiently.
Qualifications
Experience: A minimum of one year hospital pharmacy experience is preferred.
Education: Graduate of an accredited College of Pharmacy Doctor of Pharmacy (Pharm.D.) degree preferred.
Additional qualifications
Training: Registered Pharmacist (RPh) licensure in the state of Minnesota required. Advanced Cardiac Life Support (ACLS) and Basic Life Support (BLS) certification preferred. American Society of Health-System Pharmacists (ASHP) accredited residency or equivalent major healthcare experience preferred.
Special Skills: Ability to distinguish between medications is required. Ability to differentiate between sound-alike, look-alike drugs and to identify differences between drug labeling and intravenous (IV) solution coloring required.
License or certification
License to practice Pharmacy in the State of Minnesota or eligibility for licensure.
