Position descriptionLocation: Mayo Family Clinic SE, Family Medicine, Primary Care Rochester
Patient Care Type: Adult and Pediatric outpatient
Nursing Team Roles: The LPN is an integral member of the care team and involved in rooming patients for provider visits, assisting with procedures, providing immunizations and treatments during office visits, administering care on the LPN nurse calendars in a variety of aspects, providing simple focused assessments to patients across the age continuum, providing telehealth based care through electronic messaging systems in a primary care setting that is very patient centric.
The licensed practical nurse (LPN) provides nursing care to assigned patients under the direct supervision of the registered nurse (RN) or other assigned supervisor and accepts delegation from the RN/assigned supervisor in meeting the needs of the patient/family. The LPN collects data through observation and communicates information to assist the RN and/or provider in patient assessment and care planning. The LPN participates as a member of the health care team and accepts delegation from the RN/assigned supervisor and provider in meeting needs of the patient/family. The LPN delivers care in accordance with the patient care plan, policies and procedures of the organization and principles of relationship-based care. The LPN possesses excellent communication skills; is skillful in mentoring and instructing; and may participate on committees or projects, including quality improvement projects.
QualificationsGraduate of a school of practical nursing or passed LPN Boards after a defined period in a professional nursing program (e.g., some states allow RN students to take the LPN board exam after completing one to two years in the RN program). If graduation did not occur within the last two years, one year of LPN experience working in an applicable setting is required.
Additional qualificationsExcellent communication skills (verbal and written). Experience working in a team environment. Strong keyboarding and computer application skills required, prior experience with electronic medical record systems preferred. Ability to work flexible hours. Ability to adapt to unpredictable situations within the work setting. Demonstrated leadership, effective communicator, and excellent critical thinking skills.
License or certificationCurrent LPN license by applicable state requirements.
Midwest: Maintains current Basic Life Support for Health Care Providers from one of the following programs: American Heart Association or American Red Cross.
Additional specialty certification/training as required by the work area.

Compensation DetailEducation, experience and tenure may be considered along with internal equity when job offers are extended. The minimum pay rate is $21.73 per hour.

Schedule detailsMonday – Friday; Day and evening hours; 4 & 8-hour shifts between the hours of 7:30am-5pm
Will be expected to physically float to other Primary Care sites as needed. This includes but is not limited to Rochester and Kasson.
Will be expected to cross cover for non-visit care as directed by patient care needs, which may include but is not limited to virtual support.
