Responsibilities

Mayo Clinic in Rochester, MN is excited to be expanding our team and practice model, seeking a solid organ transplant pharmacist to join our team of 14 transplant pharmacist colleagues in a new 50/50 hybrid inpatient and outpatient role.  

In both inpatient and outpatient spaces, the pharmacists are responsible for questions, team meetings, quality improvement, practice management and expansion, and transition of care activities.  Providing direct patient care, while working under multiple collaborative practice agreements is part of this role. Involvement in committees, quality improvement, and precepting is expected. Research is encouraged. This transplant pharmacist will work with a variety of multidisciplinary team members to evaluate, educate, plan, monitor, customize, and re-design therapies. 

This model will rotate opposite a solid organ transplant pharmacist colleague, revolving between inpatient and outpatient solid organ transplant. Mayo Clinic transplant pharmacists provide exceptional care for kidney, pancreas, liver, heart, lung, VCA (Vascularized Composite Allotransplantation), multi-organ transplants; living donors; pediatric and adult patients. Inpatient clinical services consist of daily rounding on a multidisciplinary team, identification of drug-related problems, provision of recommendations regarding optimization of drug therapy, admission and discharge medication reconciliation, and transition of care planning. Organ or practice area preferences will be considered as able. Outpatient clinical services occur across the continuum of care, including participation in the pre-transplant evaluation & selection of candidates, participation in the immediate post-transplant phase of care, and providing care to patients during long-term post-transplant follow up.

This is a full-time position in Rochester, MN & will report to the Medication Management Services Senior Manager (outpatient). This position will rotate between outpatient and inpatient every 4 weeks. The role comprises mostly of day shifts, with an estimated 3 evening shifts and one weekend every 8 weeks (about 2 months), and one holiday per year during the inpatient portion. During outpatient time, the pharmacist will work Monday – Friday varied shifts between the hours of 7 AM – 4 PM & 8 AM – 5 PM.  



Qualifications

Job Requirements:

  • Graduate of an accredited College of Pharmacy
  • Possession of a Doctor of Pharmacy degree OR a Bachelor of Science degree and 2-years’ relevant experience

Preferred Requirements:

  • If you possess a Bachelor of Science degree, 10-years of relevant experience is preferred.
  • Completion of an accredited PGY-2 pharmacy residency solid organ transplant or an additional specialty with applicable time dedicated to transplant care provision 
  • BCTXP or relevant board certification 
  • Eligible to be a registered provider of MMS/MTM through MHCP (Minnesota Medical Assistance). If limited experience with Medication Management Ambulatory Care, completion of an accredited MTM (Medication Therapy Management) certificate program is required within 12 months of hire. 
  • Ideal candidates demonstrate excellence in teamwork, agility, innovation, communication, and forward thinking.

License or Certification:

  •  Licensed to practice pharmacy in Minnesota
