- A physician-led 24/7 virtual monitoring center
- On-demand acute medical care management, and
- Rapid-response teams to deliver supplies and services
As a critical member of a multidisciplinary team, you'll use your well-developed skills as an LPN to assess acute care patients and deliver evidence-based nursing care to patients in their homes as well as the Eau Claire hospital.
Graduate or student graduating from a school of practical nursing or passed LPN Boards after a defined period in a professional nursing program (e.g., some states allow RN students to take the LPN board exam after completing one to two years in the RN program). If graduation did not occur within the last two years, one year of LPN experience working in an applicable setting is required.
One year LPN experience within the last five years preferred. Excellent communication skills (verbal and written).
Experience working in a team environment. Computer skills required, prior experience with electronic medical record systems preferred. Ability to work flexible hours, which may include days, evenings, nights, holidays, weekends, and on-call. Ability to adapt to unpredictable situations within the work setting. Graduate of an accredited school of practical nursing preferred.
License or certification
Current LPN license by applicable state requirements. Maintains current Basic Life Support for Health Care Providers from one of the following programs: American Heart Association or American Red Cross. Additional specialty certification/training as required by the work area.
Valid driver's license, reliable transportation, the ability to transport patients and acceptable driving record as defined by policy.
Education, experience and tenure may be considered along with internal equity when job offers are extended. The minimum pay rate is $19.65 per hour.
Hours / Pay period
64 hours every two week pay period
8 and 12 hours shifts, primarily daytime hours with occasional evening and night responsibilities.
Rotating every 3-4th weekend based on the department need.
