Position description
Primary Care Rochester is seeking a Nurse Practitioner to work as part of a multidisciplinary team within Community Pediatric and Adolescent Medicine. Community Pediatric and Adolescent Medicine offers the opportunity to demonstrate leadership and creativity within the challenges of evolving health care delivery models. Our Nurse Practitioners are supported within a dynamic care team and encouraged to contribute to improving the health of our pediatric population in many ways. Our team has low turn-over, supportive physician colleagues, and strong nurse practitioner role models. You would have the opportunity to provide acute, chronic, and preventive health care services to pediatric patients in the outpatient clinic setting. The provision of care includes, but is not limited to, comprehensive evaluation, ordering and interpreting diagnostic tests, prescription of pharmacologic agents and treatments, non-pharmacologic therapies, and counseling patients. Our work is challenging and rewarding, we are experimenting with care delivery models that provide opportunity for creative healthcare solutions. We are focusing on transforming care delivery to sub-set populations; complex pediatric patient care, adolescents and those with mental health concerns, and those with social determinants of health risk because of poverty, culture/language barriers, or refugee status. Qualifications
Masters prepared nurse practitioner with current Minnesota RN and APRN license, DNP preferred

PNP preferred, FNP with strong pediatric background would be considered
