This position is in Home Health and Hospice, with responsibilities to evaluate and provide appropriate care in patient's home. There will be some travel within a selected area.
Responsible for the evaluation, planning, directing and administering of occupational therapy modalities of treatment as prescribed by a licensed physician. Administers treatments in an effort to restore function and prevent disability following injury, disease or physical disability. Assists patients to reach their maximum performance and level of functioning, while learning to live within the limits of their capabilities. The Occupational Therapist coordinates delegates and/or supervises responsibilities assigned to supportive staff (Home Health Aide, Personal Care Worker), participates in operational aspects of the department, and performance improvement activities within the department. Also provides input in formulating budget and takes appropriate action to remain in budget guidelines. Ensures that patient charges are accurate and submitted on a timely basis. Participates in all infection control, departmental equipment training and organizational safety and fire safety programs.
1-3 years of previous experience in all aspects of Occupational Therapy required. Able to meet synching requirement by obtaining the appropriate supported high speed internet access at home or the ability to obtain upon hire. Accommodations will be made for individuals who are unable to obtain. Ability to read and communicate effectively in English. Knowledge of physical medicine, occupational therapy modalities, anatomy and physiology. Strong written and verbal skills. Basic computer skills and experience with other technological devices (i.e. cell phone, etc.). Team-building skills. Strong organizational skills. Self-motivated/independent. Ability to exercise independent judgment. Practical and constructive decision-making and problem solving skills. Able and willing to work with all populations served. Able and willing to drive in all weather conditions and cover service area.
Acute Care experience preferred. Previous occupational therapy and home health experience preferred.
License or certification
Current state of Wisconsin Occupational Therapy license required. Satisfactorily passed the National Registration Examination. Current BLS certification. Valid driver's license, reliable transportation, and acceptable driving record as defined by pol
Education, experience and tenure may be considered along with internal equity when job offers are extended. The minimum salary every 2 weeks is approx $1736.96.
Monday through Friday 8 to 5. Primary job responsibilities with be in home health and hospice, but coverage throughout the region will also be needed.
Weekend rotation may be required.
