Advanced Care at Home is a care model that delivers innovative, comprehensive, and complex care to patients—all from the comfort of home via a new technology platform. Through advanced care at home, patients with conditions previously managed in a hospital will have the option to transition to a home setting and receive compassionate, high-quality virtual and in-person care and recovery services.
Advanced Care at Home is located within the Division of Hospital Internal Medicine at Mayo Clinic in Florida, which specializes in the diagnosis and treatment of multiple conditions including but not limited to hypertension, diabetes, heart failure, COPD, sepsis, and bowel obstructions in complex medical patients. Experience with the medical management of organ transplant and advanced cancer patients are encouraged. The team provides innovative, comprehensive care for both routine diagnosis as well as critically ill patients. We are an intricate part in the co-management with other specialties to ensure positive patient outcomes. In addition, we are involved with evidence based research and publishing for the advancement scientific medicine.
The Nurse Practitioner/Physician Assistant works cooperatively with MD's, other APP’s, RN's and other allied health personnel performing the following tasks in any authorized setting, including the home setting:
-Obtaining patient history
-Performing physical examinations
-Ordering and performing diagnostic and therapeutic procedures
-Formulating a diagnostic impression
-Developing and implementing a treatment plan
-Monitoring effectiveness of therapeutic interventions
-Offering counseling and education to meet patient needs
-Making appropriate referrals
-Maintains prescribing and dispensing privileges in Florida.
-Demonstrates ability to assess data reflective of patient's status and appropriately interprets information relative to patient's age-specific needs.
In addition to the above requirements, 100% of days worked require travel and involve hospital-based shifts.
Physician Assistant:
Must be a graduate of CAAHEP (Council on the Accreditations of Allied Health Education Programs) approved university program. NCCPA certification required. Must be licensed/certified or eligible to practice as PA in the State of Florida and Wisconsin.
Nurse Practitioner:
Master's degree in Nursing required. (If licensed prior to 1998, Master's degree not required by the State of Florida.) Must have current Florida Advanced Registered Nurse Practitioner (ARNP) license or eligible to obtain Florida license and Wisconsin. Certification by the ANCC or AANP in a specialty area required.
In-patient experience preferred. Must have excellent verbal and written communication skills and possess strong computer skills. Must be flexible and willing to handle extended hours and on-call responsibilities (evenings, weekends, and holidays) if need arises in the department. Minimum of 3 years of experience preferred. Experience providing care in a patient’s home or via telehealth/remote monitoring a plus.
Nurse Practitioner or Physician Assistant license to practice in the State of Florida. BLS is required. ACLS required in specific areas. For Nurse Practitioners, certification by the ANCC or AANP is required and for Physician Assistants, NCCPA certification is required.
*Must provide evidence of application for DEA certificate upon hire, obtain DEA certificate within 90 days of hire, and maintain certificate thereafter unless waived by department/division.
*Must maintain a valid driver’s license and have acceptable driving record per an annual motor vehicle record check
Days, Nights, Weekends and Holidays are expected; 12 hour shifts; schedule may fluctuate based on needs of the practice.
