Position description
The Registered Nurse (RN) Coordinator works within an interdisciplinary team to facilitate the patient plan of care throughout the continuum of care by ensuring appropriate utilization management, care coordination, resource utilization, patient report hand-offs, pre-authorization processes and documentation for patients transferring into and out of Mayo Clinic facilities. The RN Case Coordinator will function within the Mayo Clinical Nursing Professional Practice Model, which includes accountability for assessing, planning, implementing, evaluating, and communicating the patient transport/transfer plan. The RN Case Coordinator utilizes the principles of mutual respect, patient/family advocacy and provides leadership within the team of internal partners and outside agencies to facilitate best practices that achieve quality clinical, financial, and patient satisfaction outcomes. The RN Case Coordinator provides leadership through education to guide providers on safe patient transport pathways utilizing best practice initiatives, management concepts, committee work, research, and community involvement. The RN Case Coordinator bridges communication between providers, patients and families, members of the interdisciplinary team, and outside agencies to assure high-quality care that meets the patient's needs and is delivered in a cost-effective and timely manner.
Qualifications
Graduate of an Accredited nursing school at a bachelor or Master’s degree program. Minimum three years of critical care experience as an RN.
Additional qualifications
3 – 5 years of Emergency or Intensive Care Unit nursing preferred; 1-2 years of pre-hospital or inter-facility transport experience is preferred.
License or certification
Current RN license by applicable state requirements. Arizona and Florida: Maintains Basic Life Support (BLS) competency. Midwest: Maintains current Basic Life Support (BLS) for Health Care Providers from one of the following programs: American Heart Association or American Red Cross. Additional specialty certification/training as required by the work area. Advanced Cardiac Life Support (PALS). Optional preferred certifications include Neonatal resuscitation (NRP) and national certifications (CEN, CHRN and CCRN).

Compensation Detail
Education, experience and tenure may be considered along with internal equity when job offers are extended. The minimum pay rate is $34.72 per hour.
