Responsibilities
Identify and address patients’ social drivers of health in collaboration with care team members across multiple Mayo campuses and communities. Serves as a resource for the care team to provide support and solutions to address social drivers of health. Develops processes and ensures referrals to external resources enhancing patients’ ability to navigate barriers and achieve optimal health. Build and sustain network of community relationships and resources that will benefit and improve patients’ health outcomes.
**Visa sponsorship is not available for this position. Also, Mayo Clinic DOES NOT participate in the F-1 STEM OPT extension program.
Qualifications
Two years of post-secondary education and clinical/medical experience preferred. Current, continuous, and trusted member of the community. Experience with community programs or health organizations. Demonstrated skills in creative problem-solving. Possess a high level of communication skills (oral and written) necessary for assessment of health concerns and referral for appropriate service delivery. Ability to interact with health team members, patients, and members of the served communities effectively and professionally. Physical ability to perform all assigned duties; ability to physically travel to multiple Mayo sites and navigate the clinical building/environment and broader community is required. Sensitivity to, knowledge and understanding of cultural, age and financial among other diversities and varying coping styles of families under stress. Ability to work independently. Computer, spreadsheet, and word processing skills.
Additional experience:
Population-specific and/or lived experience preferred. Knowledge of population characteristics and desired outcomes is essential. Excellent communication skills (verbal and written). Experience working in a team environment as well as comfortability with working independently. Computer skills (e.g., PowerPoint, Excel, Word) required; prior experience with electronic medical record systems preferred. Ability to work flexible hours, which may include days, evenings, and weekends. Ability to adapt to unpredictable situations within the work setting. Demonstrated leadership, effective communicator, and excellent critical thinking skills. Demonstrated ability to use data for trend analysis and decision making. Also aware of cultural communications and legal and ethical implications to providing care.
