Responsibilities
The Complex Care Kidney Transplant Social Worker will provide comprehensive psychosocial support to patients undergoing kidney transplantation. This role involves working closely with the Kidney Complex Care Transplant Team to ensure that patients receive the necessary emotional, social and financial support throughout the transplant process.
Key Responsibilities
- Conduct psychosocial assessment of patients and their families to identify needs, mental health disorders, and develop care plans.
- Assessing data reflective of the patient's status and appropriately interpret information relative to the patient's age-specific needs.
- Provide counseling and support to patients and families to help them cope with the emotional aspect of transplantation.
- Provide brief behavioral health interventions.
- Collaborate with the transplant team including but not limited to physicians, surgeons, nurses, and social work colleagues to coordinate patient care and ensure a whole person approach to treatment.
- Assist patients in navigating the healthcare system, including insurance and financial assistance.
- Provide education to patients and families on transplant expectations, financial considerations, mental health needs, and financial considerations through the transplant process, including pre- and post-transplant care.
- Provide educational support to staff.
- Advocate for patient's needs within the healthcare system and community resources.
- Maintain accurate and up-to-date patient records and documentation.
- Handle difficult conversations with patients, families, and medical providers, including sensitive and at times unfavorable information and news.
- Maintaining positive working relationships with vendors and external agencies to access services for patients.
- Must use critical thinking and innovation around transplant, chronic illness, and mental health differentiation.
*This position is not eligible for visa sponsorship with the exception of the TN visa classification; Also, Mayo Clinic DOES NOT participate in the F-1 STEM OPT extension program.
Qualifications
To be considered for this position, you must have a master's degree in social work (MSW).
Additional Qualifications
- One year experience in a clinic, hospital, medical setting, or healthcare field is preferred, preferably in transplant or complex care.
- Certification in Transplant Social Work (CCTSW) or willingness to obtain certification is preferred.
- Strong understanding of the psychosocial aspects of chronic illness and transplantation.
- Knowledge of healthcare systems, insurance, and financial assistance programs.
- Knowledge of adjustment to medical diagnosis and chronic disease
- Excellent communication and interpersonal skills to interact with patients, family members/care givers, providers and allied health staff.
- Ability to work collaboratively as part of a multidisciplinary team.
- Psychosocial diagnostic skills and ability to coordinate transition of care.
- Ability to clinically assess and treat emotional mental health issues.
- Ability to diagnose under the direct supervision of a LCSW social worker.
License
Active license through Arizona State Board of Behavioral Health Service Examiners; Licensed Master Social Worker (LMSW) required.
