Social Worker - LMSW - Liver Program

The Post Liver Transplant Social Worker provides specialized psychosocial care during the post-transplant phase including substance use disorders, mental health concerns, adherence challenges, caregiver instability, mental health support, relapse prevention care, and complex case management. This role focuses on patients post transplant, however there are times you are assisting with urgent transplant evaluations and re transplant needs. during the post transplant phase. The social worker is an integral part of the multidisciplinary team and would be working with one of the largest transplant programs in the country to promote long-term transplant success and reduce preventable complications.

·  Experience working with complex medical populations, chronic illness, or transplantation.

·  Experience in substance use treatment, recovery services, motivational interviewing, or relapse prevention.

·  Interest in obtaining or maintaining substance use, addiction, or transplant-related specialty certification.

·  Passion for supporting patients through recovery, behavior change, and long-term transplant success.

Under the supervision of a social worker who meets state required standards, conducts diagnostic assessment, determines clinical interventions, facilitates crisis intervention and psychotherapy and provides education for patients and their families. Communicates effectively verbally and in writing while collaborating with multidisciplinary teams and external support systems. Participates in the development and implementation of the treatment/transition plan. Functions as a multidisciplinary team member. Functions within the research, practice, and education shields at Mayo Clinic. Complies with ethical and NASW standards for social workers in a healthcare setting including medical/legal responsibilities and serves as a resource to other allied and administrative staff within the section of Social Work and the Department of Nursing.

A MSW degree is required from an accredited Masters of Social Work Program. 

Must have an advanced knowledge of categories in mental illness, personality development, knowledge of the types of psychiatric treatment, and skills in conducting individual, group, and family therapy. Must have knowledge in adjustment to medical diagnosis and chronic disease. Strong human relation skills are needed to interact with patients, patient's family members, consultants, residents, hospital personnel, community agencies, and other resources. Must also have psychosocial diagnostic skills.

Licensure by applicable state requirements. 

• Arizona: Active license through Arizona State Board of Behavioral Health Examiners; Licensed Master Social Worker (LMSW) is required. 
• Specialty certification/training as preferred by the work area. 
• A list of approved certifications is available on Mayo Clinic intranet sites. 
• LMSW with 2 years medical experience required. 
• LCSW preferred. 
• Transplant experience preferred.
