Responsibilities
Provides medication management services in coordination with other health care professionals to deliver safe, appropriate, and cost-effective medication therapy to patients. Responsibilities may include assessment of medication therapy regimens, medication therapy management, medication order verification/entry, medication distribution, medication therapy monitoring, and consultation, documentation of medication therapy plans, and medication use education and pharmaceutical care services. Participates in collaborative practice and/or protocol-driven practice as appropriate. Additionally, pharmacists are active members in quality improvement, research activities, and education as described in the job specific competencies.
Mayo Clinic's cardiovascular surgery practice and the extracorporeal membrane oxygenation (ECMO) program provide comprehensive surgical and mechanical circulatory support care for patients with a wide range of illnesses. The cardiovascular surgery practice performs 2000-3000 operations per year for patients who travel from all over the United States and worldwide with complex surgical needs and for cardiothoracic transplantation. The ECMO program is integrated with the surgical practice and offers a wide suite of mechanical circulatory support care options to the cardiovascular surgery practice, the local medical practice by consultation including cardiac arrest, and by referral from outside facilities, encompassing nearly 200 ECMO runs per year. Both programs are academically focused on teaching, quality improvement, and high-quality scientific research. The Critical Care Pharmacist will serve alongside exceptionally talented staff and receive uninhibited support from the Department of Pharmacy, Cardiovascular Surgery and ECMO Leadership, and Mayo Clinic.
The Mayo Clinic Department of Pharmacy has a dedicated ambition for creating a patient-centered, integrated care model to advance the Mayo Clinic’s Bold.Forward. strategy with focus on People & Culture, Practice Innovation, Care Team Evolution, Discoveries & Cures. In stride with these goals, in 2023 alone, our department reached over 200 scientific publication outputs and over 80 active quality improvement projects with the Mayo Clinic Quality Academy. Nearly 250 pharmacy staff attained academic rank in the Mayo Clinic College of Medicine and are being increasingly recognized on regional, national, and international stages. Our department’s pharmacy residency has been accredited by ASHP since 1973. It has grown to become the primary training site for 21 pharmacy residents including 15 various specialty residencies, 12 longitudinal local pharmacy students, and 9 pharmacy summer interns from around the country. Our staff also regularly interface with and educate medical trainees including physician residents and fellows.
Daily pharmacist clinical services consist of robust bedside rounding as part of a multidisciplinary team, identification and intervention of drug-related issues, provision of recommendations on optimization of patient-specific drug therapy, admission and discharge medication reconciliation, transition of care planning, routine use of expansive collaborative practice agreements, response to medical emergencies including cardiac arrest and mass casualty events, and direct patient care on a rotating basis. As an institution valuing education, preceptorship and mentorship of learners is expected. Involvement in scholarly activities including quality initiatives and multidisciplinary research is encouraged and supported through extensive institutional and departmental resources.
Qualifications
- Required Qualifications
- Graduate of an ACPE-accredited Doctor of Pharmacy (Pharm.D.) program,
- OR Bachelor of Science (B.S.) and a minimum of 2 years relevant experience.
- Preferred Qualifications
- Completion of pharmacy residency and specialization
- PGY2 residency or fellowship training in critical care including experience with ECMO,
- AND/OR PGY1 residency plus 3-years in cardiovascular surgery practice including ECMO,
- OR a minimum of 5-years of cardiovascular surgery practice including ECMO experience.
- Board Certification in Critical Care (BCCCP) is strongly preferred within 1-year of hire, if not already present.
- Completion of pharmacy residency and specialization
- Other Qualifications
- Evidence of ongoing scholarly pursuits including research/quality improvement, publications, or scientific presentations, preferred.
- Relevant applicable pharmacy experience working in a progressive and collaborative practice environment, preferred.
- Experience and/or interest in teaching and/or precepting, preferred.
- Commitment to excellence, accuracy, attention to detail and teamwork required. Ability to communicate effectively, work collaboratively with others, organizes time well, practice with an agile mindset, solve problems, and work independently with minimal supervision required.
License or Certification
- Current pharmacist license by the Minnesota Board of Pharmacy, or eligible to obtain within a time frame compliant with the site’s applicable employee licensure policy. May not independently fulfill responsibilities reserved only to a pharmacist until license is obtained.
