Responsibilities
The licensed practical nurse (LPN) provides nursing care to assigned patients under the direct supervision of the registered nurse (RN) or other assigned supervisor and accepts delegation from the RN/assigned supervisor in meeting the needs of the patient/family. The LPN collects data through observation and communicates information to assist the RN and/or provider in patient assessment and care planning. The LPN participates as a member of the health care team and accepts delegation from the RN/assigned supervisor and provider in meeting needs of the patient/family. The LPN delivers care in accordance with the patient care plan, policies and procedures of the organization and principles of relationship-based care. The LPN possesses excellent communication skills; is skillful in mentoring and instructing; and may participate on committees or projects, including quality improvement projects.
We are seeking a self-motivated, flexible, and detail-oriented LPN for a dual role within our Women’s Health Clinic. This position combines remote administrative tasks with direct patient care support as needed. While most of the work will be done remotely with a high degree of autonomy, the candidate must be adaptable and ready to provide in-person care when sick calls or staffing shortages occur (often with short notice, even on the morning of). Flexibility and a positive attitude in response to evolving work demands are essential.
As a Panel Manager, responsibilities will include panel metrics with a focus on virtual tasks, while also supporting clinical operations etc.
Qualifications:
- Proven experience in administrative roles, ideally within a healthcare setting.
- Familiarity with EPIC electronic health records is preferred but not required.
- Strong computer skills with proficiency in Microsoft Office Suite and other tools.
- Ability to work independently, managing time effectively with minimal supervision -proactive mindset with a willingness to take initiative.
- Flexibility to handle last-minute work changes with a positive attitude.
- Experience in ambulatory clinics -women’s health, family medicine, or similar healthcare environments preferred.
- Trustworthy, with the ability to maintain patient and department confidentiality.
Key Responsibilities:
Panel Management/Administrative Duties:
- Perform chart reviews through EPIC to prepare for Family Medicine visits.
- Support the clinical team with a variety of administrative tasks and other duties as assigned.
- Seek opportunities to improve department processes and efficiencies.
Direct Patient Care (PRN/As-Needed) for Breast Clinic, Executive Health, and Women’s Health:
- Provide in-person support for patient visits, including rooming patients- provide exceptional service with a compassionate, professional demeanor for all patients.
- Utilize EPIC for documentation, patient tracking, and coordinating care.
- Support Care Teams with real-time needs and requests.
Qualifications
Qualifications:
- Graduate of a school of practical nursing or passed LPN Boards after a defined period in a professional nursing program (e.g., some states allow RN students to take the LPN board exam after completing one to two years in the RN program).
- If graduation did not occur within the last two years, one year of LPN experience working in an applicable setting is required
- One year LPN experience within the last five years preferred.
- Excellent communication skills (verbal and written).
Experience working in a team environment. - Computer skills required, prior experience with electronic medical record systems preferred.
- Ability to work daytime hours, flexibility may be required to meet staffing needs.
- Ability to adapt to unpredictable situations within the work setting.
- Graduate of an accredited school of practical nursing preferred.
License and Certifications:
- Current LPN license by applicable state requirements.
- Maintains current Basic Life Support (BLS) for Health Care Providers.
- Additional specialty certification/training as required by the work area.
