Responsibilities
Assist individuals and communities to adopt healthy behaviors. Conducts outreach for medical personnel or health organizations to implement programs in the community that promote, maintain, and improve individual and community health. Provides information on available resources, social support, and informal counseling, Advocates for individual and community health needs, and provides services in a setting where there is low potential for exposure to blood and other high risk body fluids, communicable diseases and hazardous materials, chemicals and waste. May collect data to help identify community health needs. Implements the care plan under the supervision of appropriate patient care team members per MN state requirements and reports findings to care team partners, educators, and other healthcare providers.
Qualifications
Completion of Community Health Care Worker certificate program from an accredited program required.
Two years post-secondary education and clinical/medical experience preferred. Current, continuous and trusted member of the community. Experience with community programs or health organizations. Demonstrated skills in creative problem-solving. Ability to go door-to-door to connect with the residents of assigned community on a weekly basis. A high level of communication skills (oral and written) necessary for assessment of health concerns and referral for appropriate service delivery. Ability to effectively and professionally interact with health team members and consumers (e.g. members of the community).
Physical ability to perform all assigned duties; able to stand and walk approximately 75% of the work time. Knowledge of the needs of low-income families. Sensitivity to cultural diversity and varying coping styles of families under stress. Ability to work independently. Computer, spreadsheet and word processing skills.
Population specific experience preferred. Knowledge of population characteristics and desired outcomes. Excellent communication skills (verbal and written). Experience working in a team environment. Computer skills (e.g. PowerPoint, Excel, Word) required prior experience with electronic medical record systems preferred. Ability to work flexible hours, which may include days, evenings, and weekends. Ability to adapt to unpredictable situations within the work setting. Demonstrated leadership, effective communicator, and excellent critical thinking skills. Demonstrated ability to use data for trend analysis and decision making.
Also aware of cultural communications and legal and ethical implications to providing care.
Certification or degree from accredited Community Health worker program. Basic Life Support (BLS) is not required but preferred. If providing face to face services across state lines, licensure/certification for applicable states is required.
Visa sponsorship or transfer is not available for this position. Mayo Clinic does not participate in the F-1 STEM OPT extension program.
