Responsibilities

Responsibilities include medication management services in coordination with other health care professionals to deliver safe, appropriate, and cost-effective medication therapy to patients. Responsibilities include assessment of medication therapy regimens, medication therapy management, medication order verification/entry, medication distribution, documentation of medication therapy plans, and medication use education and pharmaceutical care services. Additionally, pharmacists are active members in quality improvement, research activities, and education. Some patient areas covered include OR/anesthesia, outpatient/ambulatory surgery, emergency department, outpatient dialysis, labor and delivery, and outpatient infusion therapy center.

This position rotates every 3rd weekend and covers two of six observed holidays per year. Salary is based on experience and training. 



Qualifications

  • Graduate of an accredited College of Pharmacy.  Doctor of Pharmacy (Pharm.D.) degree or equivalent experience.  Equivalent experience includes at least two years of relevant pharmacy practice experience.  
  • Computer experience.  Commitment to excellence, accuracy, attention to detail and teamwork.  Ability to communicate effectively, work collaboratively with others, organize time well, solve problems, and work independently with minimal supervision.
  • The following are preferred:  PGY-1 residency and/or previous hospital experience. Experience of working in a large, progressive academic medical center.  History of scholarly activities including publication and scientific meeting presentations.  Experience and / or interest in teaching.  

License or Certification

  • License to practice Pharmacy in the State of Minnesota or eligibility for licensure.
