Position description
The Adult Medical RN Care Coordinator will work with patients with chronic conditions as they transition from hospital to their home setting and follow the patient for up to 6 months. The RN care coordinator will complete one home visit per patient to ensure a safe transition from hospital to home. Other patient interactions will be through office visits, telephone and patient online portal. The focus of the care provided will be on coaching and skills transfer to increase the patient/care giver’s capacity to manage chronic conditions to reduce preventable re-hospitalizations and improve patient/caregiver quality of life.

The Registered Nurse (RN) Care Coordinator works within an interdisciplinary team to coordinate care for a defined population of patients. The goals of care include supporting improvement in health outcomes, empowerment of patients and families to engage in health seeking behaviors and disease prevention, and supporting self-management of chronic conditions. Care is provided in a holistic, patient-centered approach across settings spanning the continuum of care, with an emphasis on assisting patients to seek an appropriate level of service when needed and reducing the overall cost of care.

The RN Care Coordinator will function within the Mayo Clinic Nursing Professional Practice Model, which includes accountability for planning, implementing, evaluating and communicating all phases of nursing care for an assigned population of patients. The ANA Nursing: Scope and Standards of Practice provide a basis for the practice of the RN. The American Academy of Ambulatory Care Nursing Scope and Standards of Practice for Registered Nurses in Care Coordination and Transition Management (2016) are reflected. A subset of employees may be required to drive their personal vehicle as a part of responsibilities of their role.
BSN degree required.

The degree must be accredited through the Accreditation Commission for Education in Nursing (ACEN), Commission on Collegiate Nursing Education (CCNE) or National League for Nursing (NLN)].

Three years of RN experience required.

If driving is required, a valid driver's license and acceptable Motor Vehicle record will be required.
Additional qualifications
Three years current adult-care experience in ambulatory or acute care preferred.

Experience with adult patients living with chronic health conditions preferred.

Excellent communication skills (verbal and written).
Experience working in a team environment.
Computer skills (e.g. PowerPoint, Excel, Word) required, prior experience with electronic medical record systems preferred.
Ability to work flexible hours, which may include days, evenings, and weekends.
Ability to adapt to unpredictable situations within the work setting.
Demonstrated leadership, effective communicator, and excellent critical thinking skills.
Demonstrated ability to use data for trend analysis and decision making.

Internal applicants should submit their three most recent performance appraisals.
License or certification
Current MN RN license

Maintains current Basic Life Support for Health Care Providers from one of the following programs: American Heart Association, American Red Cross or the Military Training Network.

Additional specialty certification/training specific to care coordination as required by the work area.

Education, experience and tenure may be considered along with internal equity when job offers are extended. The minimum pay rate is $30.86 per hour.

This Registered Nurse (RN) Care Coordinator will be working in the specialty of Adult Medical Care. This position will sit in Albert Lea, but will provide coverage for other Primary Care locations as needed.
